Birth & Death Records

We are open to assist funeral homes Monday - Friday, 8:00 am - 4:30 pm. Our death certificates are $22.00 per copy, and burial permits are $3.00. You may use a form of your own, or print and fill out our form. When filing a death certificate, make sure they are complete and error free (no cross-outs, white-out, write overs, erasures). Please remember that if the decedent served in the military, their family is entitled to a free, certified copy of the death certificate. You may fill out and print this

form to get the veteran's copy. If the death certificate you are filing has "pending" listed in the cause of death area, we will mail the supplemental certificate to you when we receive it.

In order to file a certificate or an affidavit, you can come in to our office or mail it to us. We process certificates and affidavits the day that receive them.
