Adult Walk-in Clinics (for adults 19 years of age or older)

Mondays and Fridays from 1:30 p.m. until 4:00 p.m.
Tuesdays and Thursdays from 8:30 a.m. until 11:30 a.m.

Homebound Seasonal Flu Vaccination Services

Homebound older adults may request to receive a seasonal
flu shot at their home by calling Jean Daniel at 330-662-0452.

Pediatric Seasonal Flu Clinic Information

Pediatric seasonal flu vaccine appointments are available Monday through Friday
from 8:00 a.m. until 4:00 p.m. Call 1-888-723-9688 (Option 1) for an appointment.

Pediatric seasonal flu shots, nasal spray, and thimerosal-free vaccine will be available.
Pediatric seasonal flu vaccine is $10 for Medina County children and $14.50 for
out-of-county children. We accept traditional Medicare, Medicaid, and most Medicare
HMO plans. Please bring all insurance cards. The parent’s photo ID is required.

Cost

Seasonal flu vaccine is $25 for Medina County adults and $30 for out-of-county adults.
Pneumonia vaccine is $60 for Medina County residents and $65 for out-of-county residents.
Medicare pays for the flu and pneumonia vaccine for adults over 65 years of age.
We accept traditional Medicare and most Medicare HMO plans. Please bring all insurance
cards. A photo ID is required for anyone getting a flu shot.

Flu Vaccine Consent Forms

For your convenience, you may print the following forms according to your age and bring them
with you to a Medina County Health Department seasonal flu clinic. If you are unable to print out
your form, we will provide forms and pens at each clinic site.

Pediatric Form

Adult 19-49 years of age Form
Adult 50-64 years of age Form
Adults 65 years of age or older Form: This year our vaccine provider and Medicare are requiring
a special form which will be provided to you at our clinics. We will assist you with completing it.
