Office hours: Monday-Friday 8:00 a.m. - 12:00 p.m. and 1:00 p.m. - 4:30 p.m.

Services are provided by appointment. Walk-ins will be accommodated as the schedule permits. To schedule and appointment for clinic services please call 330-723-9688 or 1-888-723-9688 and select option 1.
