Important Facts About WIC

Who can qualify for WIC?
  • Pregnant women, women breastfeeding an infant younger than 12 months of age, women who had a baby within the last six months, infants and children up to five years of age
  • Participants must be seen by a registered dietitian in the WIC office
  • Must be a resident of the state of Ohio currently living in Medina County
  • All WIC participants must also meet the WIC income guidelines of <185% Federal Poverty Income Guidelines. Income guidelines for WIC are listed on the Ohio WIC website
 

 

 

 
If you think you could qualify for WIC service, call the WIC office at 330-723-9629 or toll free at 1-888-723-9688, option 4. Please have your most current paycheck in hand when you call. WIC staff will be able to pre-screen your income qualifications to determine if you are eligible for WIC. If you qualify, your first appoinment will be scheduled for you by a WIC staff member. All participants must have an appointment to receive WIC services.

 

 

 

 

All participants must have an appointment to recieve WIC services. When you come to your appointment please bring:
  • WIC program identification card (the yellow booklet you take to the store)
  • Completed forms
  • Your child/children
  • Proof of income (current pay stubs, child support, or Medicaid card)
  • Shot records
  • Proof of address
  • Proof of pregnancy if you are pregnant
 
Appointments may take 1-2 hours
 
  • Height, weight, and iron will be measured
  • You will speak to a dietitian to complete your nutrition and health assessment. Nutrition counseling will be provided at this time
  • You will learn about additional resources in the community
  • If all WIC qualifications are met and you bring all required documentation you will receive WIC coupons to purchase a supplemental food package
  • You will learn how to use the WIC program
  • Appointments may take 1-2 hours
