Physicians please be aware beginning October 1, 2012 WIC will no longer need a RX for Similac Sensitive or Similac for Spit Up. However, infants receiving these formulas will have to be evaluated for appropriate use by a Registered Dietitian at WIC. Please read the letter from The Ohio Department of Health explaining the new policy changes with formula use and new formula on the WIC program.

Ohio WIC Prescribed Formula and Food Request - Fill in prescription form for doctors to complete. Allows WIC to provide coupons to WIC participants for specialized formula covered through the WIC program. Participants must have a qualifying medical diagnosis to receive a specialized formula through WIC.

Enfamil AR will NO longer be available on the WIC program beginning September 1, 2012.

WIC is using the Centers for Disease Control and Prevention (CDC) birth to 24 months growth charts based on the World Health Organization (WHO) growth charts and the CDC Body Mass Index (BMI) charts for children 2 years and older. For more information please read the ODH letter to physicians.
