Please read the letter from The Ohio Department of Health explaining the WIC policy for formula use.

Ohio WIC Prescribed Formula and Food Request - Most current WIC prescription form with effective date of October 1, 2014. Allows the WIC program to provide coupons to WIC participants for specialized formula covered through the WIC program afer being completed by a physician. Participants must have a qualifying medical diagnosis and be evaluated by a WIC Health Professional to receive a specialized formula through WIC.

Physicians: Please read the letter from ODH discussing the upcoming changes for Similac Sensitive and Similac for Spit Up.
Changes to WIC FORMULA: Similac Sensitive and Similac for Spit Up can be offered through WIC with a completed WIC RX form and previous trial on Similac Advance. An infant receiving this formula will need to be evaluated by a registered dietitian at the WIC office before receiving Similac Sensitive and Similac for Spit Up. To learn more about Similac for Spit Up click here.

WIC is using the Centers for Disease Control and Prevention (CDC) birth to 24 months growth charts based on the World Health Organization (WHO) growth charts and the CDC Body Mass Index (BMI) charts for children 2 years and older. For more information please read the ODH letter to physicians.
